《恋恋笔记本》是一部爱情故事，讲述一位神秘的老人为住在养老院的老妇人轻声读着自己那褪色的笔记本中的日记开始。20世纪40年代初期，年少美丽的艾丽跟随家人来到北卡罗来纳州度假。在一次狂欢派对上，她结识了当地男孩诺亚，两人很快坠入爱河。然而，身处上流社会的艾丽父母对穷小子诺亚十分看不起，将艾丽带回纽约并阻断两人的信件来往。一年未收到艾丽回信的诺亚参加了第二次世界大战并成为一名军人。艾丽也在穿上嫁衣时意外看到报纸上关于诺亚的新闻。两人在最初相恋的小镇上再次相遇……故事最初的老妇人便是当年的艾丽，每当她听到日记中的字句，脸上就会迸发出异样的神采，仿佛再次回到那段激情燃烧的岁月，而向她讲述那本笔记本里的故事的老头，就是诺亚。
