游戏要求玩家将所选定的病原体散布到世界各地，从而制造一场超级瘟疫，最终让全人类死于该传染病。而在病原体传播和感染的过程中，玩家需要不断修正其传染性和抵抗力，以应对来自世界各国政府及科学家们的反击措施。初始版本下游戏提供了细菌、病毒、真菌、寄生虫、朊病毒、纳米病毒以及生物武器七种基础病原体，在后续更新版本中补充了虚构的特殊病原体Neurax蠕虫、Necroa病毒、猩猩流感和暗影瘟疫。
